If you do not have insurance coverage or know that your procedure is not covered, please make payment arrangements with the front office prior to your day of surgery.
Creekside Surgery Center will automatically process your surgical claim with your insurance provider. Remaining balances are the insured’s responsibility. These will include: the amount remaining to meet your deductible and the balance not covered by insurance, unless otherwise arranged. The insured is required to make arrangements for this amount before surgery.
At the time of registration, you will need: insurance cards, driver’s license, deposits, Medicaid coupons, sticker, or Denali Kid Care card (if using Medicaid). If registration is done over the phone or on the day of surgery, please bring these items with you.
You will be receiving separate bills from your surgeon, anesthesia, Creekside, and pathology if a specimen is sent.
